Aboriginal Diabetes Initiative

Serving the communities of Wabauskang and Lac Seul First Nations. Providing culturally relevant diabetes prevention and management programs focused on nutrition, lifestyle, and community well-being.

Aboriginal Healing & Wellness Strategy Programs

Aboriginal Health Planning Authority (AHPA)

The Aboriginal Health Planning Authority program is responsible for planning related to community, regional, secondary and tertiary health services necessary to support the implementation of the Aboriginal Healing and Wellness Strategy (AHWS).

Community Wellness Worker Program (CWW)

The Community Wellness Worker program provides family violence services, referrals, support and case management to clients to address and respond to existing and emerging health, healing and wellness issues or violent situations. These community based workers also provide prevention/awareness activities as well as liaise and networking with other health and social service agencies. Each affiliated First Nation has one full time community based worker with PFNF providing off site supervision and support.

Community Development Support Worker Program (CDSW)

The Community Development Support worker program is responsible to develop or enhance skills and capacity for the CWW program to achieve desired outcomes. It also provides direct assistance with program planning and reporting as well as supports the implementation and management of AHWS funded programs.

Training

Coordination and implementation of training requirements for Community Wellness Workers as identified.

Aboriginal Healthy Babies/Healthy Children (AHB/HC)

The AHB/HC program is a prevention and early intervention strategy of services for families with children from prenatal to six (6) years of age. The heart of the AHB/HC programming is preparation for parenting (preconception component), getting ready for the birth of baby (prenatal care), and taking care of the baby (postnatal care). Each First Nation has either a ¼ to a full time position at the community level with PFNF providing off site supervision and support.

This program provides outreach services to First Nation clients living with Diabetes in the area of:

  • Diabetes education, monitoring and screening.

  • Diabetes compliance, preventions and intervention services.

  • Networking and liaison with the Dryden Regional Health Centre and other programs as applicable.

Fetal Alcohol Spectrum Disorder (FASD) Program

The overall goal of the FASD/CNP is to provide education, prevention and awareness activities through creative, interactive and engaging programming that is culturally competent and appropriate.

The FASD community based programming and activities:

  • To provide education, prevention and awareness of FASD.

  • To promote and provide education on the importance of healthy and proper Child Nutrition.

  • To develop culturally appropriate educational resources.

  • To provide one-on-one education and support to clients and families directly and indirectly affected by FASD.

  • To coordinate referrals specialized services.

  • To provide peer counseling and advocacy on FASD and Case Management

Home & Community Care Nurse

The focus of the Community Health Program is to provide a broad range of nursing services to Wabigoon and Wabauskang First Nations. The goal of all community health activities strive to enhance overall health and well-being of both individuals and the community as a whole. All community-based programming promotes health and aims to prevent injury and illness throughout the lifespan. Services aim to increase access and promote maternal, infant, child, school, adolescent, adult and elder health within our local communities. In conjunction with these services, the Home and Community Care Program focuses on identifying individual needs of home care services. The goal of this program is to assist individuals to maintain independence, optimize level of wellness and ability to remain in their own home and community for as long as possible. These services are closely linked with the community, other community services and external health service providers to ensure quality care.

Blending traditional wisdom with contemporary care to support the mental health of our Dryden Area First Nations community.

Nita Gonéb Healing Lodge

A sanctuary for recovery and spiritual growth, offering land-based healing and traditional Ojibwe wellness practices.

Mental Wellness Programs

Holistic support services including clinical counseling and community-based mental health awareness.

Crisis Support

We are here to help support your community when there is a crisis. Crisis is self determined and often is a result of a sudden death or loss in the community. We will hold debriefs, sharing circles or meet with individuals or families one to one.
