Paawidigong First Nations Forum Diabetes Strategy Nurse is responsible for providing a variety of holistic, comprehensive, preventative and education awareness services on diabetes that also includes the promotion and incorporation of traditional knowledge and the adoption of healthy lifestyles. This includes a monitoring plan for newly diagnosed clients, individualized to meet the needs, provide follow up, teaching and support until the client is able to self-manage.

SUMMARY OF KNOWLEDGE & SKILLS:
- An approved Canadian University Baccalaureate/Bachelor’s degree in nursing
- Valid CPR certification – level HCP (Health Care Provider)
- Current certificate of registration from the College of Nurses of Ontario (CNO)
- Minimum three years nursing experience
- Certification in Advanced Foot Care or willingness to acquire is required.
- Ability to preform assessments using validated screening tools while planning care to meet the client’s health needs.
- Ability to perform diabetes nursing skills as per care plan, during client care encounters such as insulin teaching, glucose meter & blood pressure measurement and or foot care.
- Awareness of Registered Nurses Association of Ontario (RNAO) initiatives that promote best evidence-based practice.
- Ability to work with community members, health professionals, and co-workers as a member of a multidisciplinary team
- Proficient Computer Skills
- Strong Interpersonal communication skills
- Ability to show initiative and self-direction.
- Proven planning, time management, organization, and coordination skills
- Knowledge, respect and understanding of First Nations language, culture and traditions is considered an asset.
- Successful completion of a criminal record check including a vulnerable sector check.
- Travel is a requirement of this position therefore, Valid Driver’s License with good driving abstract/record required to be insured under company vehicle.
